Job summary

Loyola Medicine, a member of Trinity Health, seeks a Transplant Coordinator to oversee kidney transplant patients pre- and post-transplant. You will act as the main liaison among patients, families, physicians, surgeons, and care teams to ensure smooth evaluation, education, and ongoing support.

Responsibilities include coordinating transplant evaluations, reviewing labs, managing waitlist data with UNOS, and ensuring regulatory and insurance compliance.

Qualifications

• Associates degree required; RN licensure in Illinois.
• CPR certification required.
• 1–2 years of nursing experience; transplant/dialysis experience preferred.
• 3 years of nursing experience; transplant/dialysis highly desirable.

What you’ll do:

The Transplant Coordinator oversees and coordinates the care of kidney transplant patients throughout the pre- and post-transplant journey. This role serves as the primary liaison between patients, families, physicians, surgeons, and multidisciplinary care teams to ensure seamless evaluation, education, treatment planning, and ongoing support.

Responsibilities include managing transplant evaluations, reviewing diagnostic and laboratory results, coordinating patient care and medication management, maintaining transplant waitlist information with UNOS, and ensuring compliance with regulatory and insurance requirements. The coordinator educates patients and families on all aspects of the transplant process, provides clinical support and triage, facilitates communication among stakeholders, and supports quality improvement initiatives. This position also maintains accurate medical records, prepares reports and documentation for regulatory agencies, and collaborates across departments to deliver high-quality transplant care.

What you'll need for this job:

• Minimum Education Required: Associate degree
• Preferred: Bachelor's Degree Specify Degree(s): Nursing
• Minimum Experience Required: Required: 1-2 years of previous job‑related experience
  Preferred: 3-5 years of previous job‑related experience
  Details: Three years of professional nursing experience; at least 1 year of previous transplant, outpatient, or dialysis experience highly desirable

Licensure/Certifications: Required:

• CPR (Cardiopulmonary Resuscitation)
• Current Registered Nurse License State of Illinois

Preferred:

Specialty certification in clinical or functional area of nursing granted by a national nursing organization.
Other: National Certification as Transplant Coordinator or other related specialty preferred.

Compensation:

Pay Range: $39.48- $65.14 per hour

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
