Qualifications

Responsibilities

Screen referrals for appropriate program options.
Establish and maintain relationships with participants, referral agencies, and team members.
Ensure all paperwork is current, accurate, and timely; including case notes and service reports.
Provide hands-on training, direct supervision, and support at various work locations.
Gather data needed for reporting, including work behaviors and productivity.
Assist with orientation of new referrals and developing intake materials.
Assist participants in achieving the highest level of independent functioning while receiving services.
Perform job and task analysis and monitor participant progress toward goals.
