Qualifications

• High school diploma or GED required, or equivalent combination of education and experience.
• Six months related experience and/or training acceptable in lieu of diploma.

Responsibilities

• Assist with screening of referrals for appropriate program and service options.
• Establish and maintain good working relationships with participants, referral agencies, and team members.
• Ensure paperwork and related activities are current, accurate, and in compliance, including case notes, service reports, and time studies.
• Provide hands-on training, supervision, and support to participants at various work locations.
• Gather data needed to fulfill reporting requirements, including work behaviors and productivity.
• Make recommendations to supervisory staff on policy or management changes.
• Assist with orientation of new referrals.
• Support participants to achieve the highest level of independent functioning while receiving services.
• Review intake, service plans, and supporting documents; assist in developing them as requested.
• Perform job and task analyses and monitor participant progress toward goals.
