Job summary

CVS Health / Signify Health is seeking a Full-Time Physician to provide in-home health evaluations across a defined coverage area. The role involves traveling up to 100% of the time, conducting one-on-one visits lasting about 45 minutes, reviewing medications and medical history, performing a physical evaluation, and ordering or coordinating diagnostic tests as needed.

MDs and DOs must hold active licenses in coverage areas or be able to obtain multi-state licensure.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Conduct in-home health evaluations with patients, including medication and medical history review and physical examination.
• Perform diagnostic tests as ordered by health plan (e.g., spirometry, etc.) and utilize secure clinical workflow.
• Travel across defined coverage area, up to 100% of the time, including potential overnight stays and flights.
• Provide Signify Health services per program requirements, including preventive services.
• Collaborate with health plan teams and primary care physicians to coordinate care.
• Maintain licensure and comply with state requirements and coverage areas.
