Job summary

Signify Health, part of CVS Health, seeks a Full-Time Physician to perform in-home health evaluations across defined coverage areas. The role involves traveling up to 100% of the time, conducting comprehensive visits with patient health histories, physicals, and optional diagnostics, while coordinating with health plans and Signify Health teams.

MDs and DOs must hold active licenses in coverage areas and be able to obtain multi-state licenses as required.

Qualifications

• Active, unrestricted medical license in coverage area(s).
• Ability to obtain and maintain multi-state licenses as required.

Responsibilities

• Bring your heart into every visit and connect with patients in their homes.
• Perform comprehensive in-home health evaluations including med history and physicals.
• Carry out diagnostic tests if ordered by the health plan.
• Travel to defined coverage areas; stay updated on state licensure.
• Collaborate with Signify Health colleagues and health plans.
• Conduct virtual visits when needed.
