Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations across a defined coverage area. The role involves traveling up to 100% of the time, performing med history reviews and physical evaluations in members' homes.

You will work within a national clinician network, coordinating care with health plans and PCPs using an iPad-based workflow. Licensure across required states is essential.

Qualifications

• Active, unrestricted license(s) in coverage area(s)
• Able to obtain and maintain multi-state licenses in coverage area, as required

Responsibilities

• Bring your heart to every visit and join a national network of clinicians improving health outcomes
• Visit millions nationwide to connect members with the right care
• Conduct in-home evaluations using a streamlined workflow on an iPad
• Provide diagnostic & preventive services as ordered by health plans
• Travel locally and to licensed states as required
