Job summary

Signify Health, part of CVS Health, seeks a Full-Time Physician to provide in-home health evaluations across a defined coverage area. Travel up to 100% of the time; involves in-home visits with 45-minute assessments and multi-state licensing as required.

You will join a national network of clinicians, delivering diagnostic and preventive services and coordinating care with health plans and PCPs to support patients in their homes.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage areas as required.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
• Be flexible to travel locally and within licensed states.
