Signify Health is seeking a Full-Time Physician to provide in-home health evaluations across a defined coverage area. The visit lasts about 45 minutes, including medication and medical history review, a physical evaluation, and optional diagnostic tests as ordered by the health plan.

You will travel up to 100% and use an iPad-based workflow. Holders of active medical licenses in coverage areas and ability to obtain multi-state licenses are required.

Qualificações

• Active, unrestricted medical license in the coverage area(s).
• Able to obtain and maintain multi-state licenses as required.

Responsabilidades

• Travel to patient homes for in-home health evaluations.
• Conduct 45-minute visits including medication and medical history review and physical evaluation.
• Perform diagnostic tests when ordered by health plan (e.g., spirometry, etc.).
• Utilize iPad and clinical workflow; do not prescribe medicines or modify current treatment regimens.
