Signify Health is seeking a Full Time Physician to provide in-home health evaluations, traveling across a defined coverage area. Visits last about 45 minutes, including medication history, physical exam, and potential diagnostic tests as ordered by the health plan.

You’ll use an iPad and a clinical workflow while not prescribing or altering current regimens. You must hold an active, unrestricted MD or DO license with ability to obtain multi-state licenses as required.

Qualifications

• Active, unrestricted licenses in coverage area.
• Able to obtain and maintain multi-state licenses as required.
• Willingness to travel up to 100% and work in patients’ homes.

Responsabilités

• Conduct in-home health evaluations (~45 minutes) including medication and medical history review and physical examination.
• Provide Signify Health services as indicated, including diagnostic tests where applicable.
• Conduct virtual visits when needed and coordinate with health plans for referrals.
