Job summary

Signify Health Medical Associates, PLLC is seeking a Full Time Physician to provide in-home health evaluations, engaging with people in the comfort of their own homes to bridge gaps in care.

You will travel across a defined coverage area (up to 100%), visit members in their homes, perform physical evaluations and review medical history, while using an iPad and clinical workflow. Licensure in multiple states may be required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Provide in-home health evaluations across a defined coverage area.
• Conduct 45-minute visits including medication and medical history review and a physical evaluation.
• Travel up to 100% of the time with potential overnight stays.
• Use an iPad and clinical workflow; no prescribing or ordering of tests required.
• Provide Signify Health services including diagnostic & preventive services as indicated.
• Travel locally and within licensed states.
