Job summary

Your Health is seeking a Registered Nurse to serve patients across the Lexington, West Columbia, Chapin, Newberry and Batesburg-Leesville area. The Nurse Case Manager will travel to patients’ homes and facilities, coordinating with providers and initiating telemedicine visits to ensure quality, compliant care.

Ideal candidates are licensed RNs with at least three years of clinical experience, strong communication skills, and the ability to work independently in diverse settings.

Qualifications

• RN license in good standing with the state board.
• Minimum of three years of clinical experience.
• Experience in community or home-based settings preferred.
• Strong written and verbal communication skills.
• Ability to coordinate with interdisciplinary care teams.

Responsibilities

• Facilitate receiving and review of patient medical records.
• Enroll patients in Care Management and initiate care plans.
• Coordinate care with providers and facilities.
• Travel to patients’ homes and care facilities as needed.
• Document Care Management activities accurately.
