Job summary

Your Health Organization in South Carolina is seeking a Registered Nurse with hospice experience to serve patients in the Blythewood and Winnsboro areas. The Nurse Case Manager will travel to patients' homes and facilities, coordinating with providers and initiating telemedicine visits as needed.

This is a full-time position with 12-hour shifts, requiring daily travel. A valid RN license and reliable transportation are essential to deliver high-quality care across care settings.

Qualifications

• Must be a licensed RN in good standing.
• Minimum 3 years clinical experience.
• Experience in community settings preferred.
• Strong written and verbal communication skills and collaboration with care teams.

Responsibilities

• Visit patients in homes or facilities to assess needs and coordinate care.
• Coordinate with providers and telemedicine visits as needed.
• Enroll patients in Care Management and develop Plan of Care.
• Reconcile medications and coordinate with interdisciplinary teams.
• Document Care Management activities accurately.
