Job summary

Mainline Health Systems Inc. in Arkansas is seeking a Community Health Worker to join the care team under the Director of Care Management. The position involves meeting patients and community partners, coordinating care, and supporting health program enrollment and outreach.

Qualifications

• A high school diploma or equivalent required; Associate’s degree or higher in health or human services preferred.
• Experience in the substance abuse and/or mental health field preferred.
• Functional knowledge of Microsoft Word, Excel, Outlook, and EMR Systems.

Responsibilities

• Promote customer-driven excellence as a driver of organizational services and goals.
• Meet with patients, individuals, and/or community organizations as needed to support the patient and program development.
• Ensure efficient tracking and documentation of all activities and interaction with each patient.
• Participate in community events and outreach efforts to raise awareness and develop relationships.
• Assist with coordination of care with the clinical team, other Community Health Workers, and/or community groups based on the needs of the patient.
• Participate in home visits to monitor their progress and assess any needs.
• Maintain contact using variety of mediums to check on their status, progress, barriers, needs and assist with coordination of care including home visits.
• Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
• Performs all job duties in compliance with MHSI Safety and Infection Control policies.
• Assists in developing reports to meet organizational internal and external requirements.
• Assists patients with insurance outreach and enrollment.
• Will possess an Arkansas Community Health Worker certification OR will be currently seeking certification.
