Facing Forward to End Homelessness is seeking a Case Manager in Chicago. The role provides home-based case management to support residents in permanent housing, income maintenance, and skill development.

Travel between client homes is required, and staff will enter data into HMIS and maintain records. The position is hybrid, involving field work, remote tasks, and office days, with standard 9 am-5 pm hours and some evening/weekend work as needed.

Qualifications

• Provides comprehensive home-based case management to support stabilization in permanent housing.
• Home visits are a requirement for this role.
• Completes intake, assessments, service plans, and progress assessments.
• Documents all client contacts and services in progress notes.

Responsabilités

• Provides comprehensive home-based case management services.
• Complete intake, assessments, service plans, annual progress assessment, benefits assessment.
• Provide life skills instruction (financial literacy, tenant education, parenting, health/wellness).
• Collaboratively develops and updates service plans with clients.
• Documents all client contacts and services in progress notes.
