Job summary

Terros Health in Glendale, AZ is seeking a Families FIRST - Care Coordinator at the Olive Ave Health Center to support families involved with the Department of Child Safety.

You will coordinate with DCS and clinical teams, conduct in-home assessments, and connect families to behavioral health and community resources to remove barriers and promote reunification.

This full-time role requires travel outside Maricopa County and offers comprehensive benefits and generous PTO.

Responsibilities

• Accepts and manages AFF referrals outside Maricopa County.
• Outreach to referred families, coordinate with DCS Specialists, and facilitate engagement in AFF services.
• Travels throughout assigned regions to conduct in-home assessments and complete intake documentation.
• Develops short-term service recommendations and links to behavioral health, substance use treatment, recovery support, and community services.
• Coordinates ongoing AFF services through approved subcontracted providers, community partners, telehealth resources, and Terros Health programs as appropriate.
• Maintains responsibility for ongoing care coordination to align services with needs and DCS requirements.
• Develops and monitors individualized Service Plans within AFF and DCS timelines.
• Facilitates multidisciplinary meetings with patients, families, DCS Specialists, and providers to review progress and barriers.
• Participates in Child and Family Team, Adult Recovery Team, Team Decision Making, SENSE, and other meetings as required.

The Families FIRST program at Terros Health assists families involved with the Department of Child Safety. Working as part of a team, the Care Coordinator will impact the lives of families trying to overcome the barrier of substance abuse to reach family reunification and self-sufficiency. This individual will work closely with DCS, the clinical team and other providers to coordinate and help families be successful in their DCS case plans. Part of their responsibility is to provide monthly meetings to assure that goals are being set and achieved. They will also empower families by providing resources and skills to show stability in their homes. The individual will need to be professional, friendly, a self-starter, organized, and compassionate.

Under Terros Health’s policies and professional requirements, with oversight from clinical staff, the Families FIRST Care Coordinator works closely with DCS, collaborative partners and other community resources to engage patients and streamline services to remove barriers, alleviate substance use and improve the prospect of family reunification. This position reports to the Manager of Care Coordination

Essential Duties

• Accepts and manages Arizona Families FIRST (AFF) referrals assigned to Terros Health in service areas outside of Maricopa County.
• Completes initial outreach to referred families, coordinate with DCS Specialists and other involved stakeholders, and facilitates engagement in AFF services.
• Travels throughout assigned geographic regions to conduct in-home assessments, completes intake and assessment documentation, gathers required information, and determines service needs.
• Develops short-term service recommendations and coordinates timely linkage to behavioral health, substance use treatment, recovery support, and community-based services.
• Coordinates ongoing AFF services through approved subcontracted providers, community partners, telehealth resources, and/or Terros Health programs as appropriate.
• Maintains responsibility for ongoing care coordination throughout the episode of care, ensuring services align with identified needs and DCS requirements.
• Develops, implements, and monitors individualized Service Plans within required AFF and DCS timelines.
• Facilitates multidisciplinary meetings and collaborates with patients, family members, DCS Specialists, treatment providers, and community partners to review progress, identify barriers, and support service plan goals.
• Participates in Child and Family Team (CFT), Adult Recovery Team (ART), Team Decision Making (TDM), SENSE, DCS staffing meetings, and other collaborative meetings as required.
