Qualifications

• BA/BS required; Master's preferred.
• 5-7 years in healthcare environment.
• Licensure within two years of hire (Social Worker, Chaplain, or RN).
• Active Indiana license or multi-state licensure (NLC) required.
• Strong emotional intelligence and ability to navigate sensitive conversations.

Responsibilities

• Support ACP program development and broad implementation across the health system.
• Initiate and facilitate ACP conversations tailored to patient needs and beliefs.
• Assess the needs of individuals seeking ACP assistance with a person-centered approach.
• Promote ACP as a standard component of healthcare through system-wide initiatives.
• Develop and deliver staff and community education programs on ACP.
• Collaborate with healthcare teams to ensure ACP aligns with patient goals.
• Thoroughly document patient values, preferences, and decisions for care teams.
• Travel to multiple clinics across the Indianapolis area to meet patients where they receive care.
• Maintain current knowledge of evidence-based practices in advance care planning.
