IU Health is seeking an Advance Care Planning Advisor at Bloomington Hospital to lead program development and system-wide ACP implementation, guiding patient, family, and clinician conversations across the care journey. This role emphasizes compassionate, non-scripted dialogue tailored to individual values and wishes, and it involves coordinating with diverse clinical teams.

Regular travel to clinics within the Indianapolis area is required, and the Advisor reports to the Advance Care Planning

Qualifications

Bachelor's degree required; Master's degree preferred.
5–7 years of healthcare experience.
Licensure or board certification within two years of hire.
Valid Indiana license as a Social Worker, Chaplain, or RN with an active RN license or the Nurse Licensure Compact (NLC).
Strong emotional intelligence and interpersonal skills; comfortable navigating sensitive, values-based conversations without a scripted approach.
Reliable transportation for regular travel between clinic sites.

Responsibilities

Support program development and system-wide ACP implementation.
Initiate and facilitate person-centered ACP conversations.
Assess needs of individuals seeking ACP assistance.
Promote ACP as a standard component of healthcare across the system.
Develop and deliver staff and community education on ACP.
Collaborate with Clinical Ethics and other teams to align ACP with patient goals and values.
Document patient values and treatment decisions to support care teams.
Travel to multiple clinics across the Indianapolis area to meet patients where they receive care.
Maintain current knowledge of evidence-based ACP practices.
