Job summary

IU Health seeks an Advance Care Planning Advisor to drive program development and system-wide ACP implementation across clinics. You will promote person-centered conversations, document patients' values, and collaborate with Oncology, Clinical Ethics, and Primary Care teams.

You will travel between Indianapolis area clinics to meet patients where they receive care, applying evidence-based practices and providing staff education to improve ACP understanding and integration into care planning.

Qualifications

• Bachelor's degree required; Master's preferred.
• 5-7 years of experience in the healthcare field.
• Requires licensure or board certification within two years of hire.
• Valid Indiana license as Social Worker, Chaplain, or Registered Nurse with an active IN license or the Nurse Licensure Compact (NLC).
• Strong emotional intelligence and interpersonal skills; comfortable navigating sensitive, values-based conversations without a scripted approach.
• Reliable transportation for regular travel between clinic sites.

Responsibilities

• Support ACP program development and broad system-wide implementation across IU Health.
• Initiate and facilitate ACP conversations tailored to individual patient needs and beliefs.
• Assess the needs of individuals seeking ACP assistance with a person-centered approach.
• Promote ACP as a standard component of healthcare through system-wide initiatives.
• Develop and deliver staff and community education programs to improve ACP understanding.
• Collaborate with Clinical Ethics and other teams to ensure ACP aligns with patient goals.
• Thoroughly document patient values and treatment decisions to support care teams.
• Travel to multiple clinics across the Indianapolis area to meet patients where they receive care.
• Maintain current knowledge of evidence-based practices in advance care planning.
