Job summary

Indiana University Health System in Bloomington, IN is hiring an Advance Care Planning Advisor to lead system-wide ACP efforts and educate staff across the health system. The role focuses on person-centered conversations and documenting patient values to guide future care decisions.

The ACP Advisor will travel between clinics around Indianapolis, partnering with Oncology, Clinical Ethics, and Primary Care teams to implement best practices and ensure ACP reflects patient wishes throughout care

Qualifications

• Bachelor's Degree required; Master's preferred.
• 5-7 years of healthcare experience.
• Licensed or board-certified within two years of hire.
• Indiana license as Social Worker, Chaplain, or RN with active Indiana RN license or NLC.

Responsibilities

• Support program development and broad ACP implementation.
• Initiate and facilitate ACP conversations tailored to patient needs.
• Assess ACP needs with a person-centered approach.
• Promote ACP as a standard component across the health system.
• Develop and deliver staff/community education on ACP.
• Collaborate with Clinical Ethics and care teams to align with patient goals.
• Document patient values and treatment decisions for care teams.
• Travel to multiple clinics across the Indianapolis area.
• Maintain current knowledge of evidence-based ACP practices.
