Qualifications

• Bachelor's degree required; Master's degree preferred.
• 57 years of healthcare experience.
• Requires licensure or board certification within two years of hire.
• Valid Indiana license as Social Worker, Chaplain, or RN with active Indiana RN license or NLC.

Responsibilities

• Support ACP program development and system-wide implementation across IU Health.
• Initiate and facilitate ACP conversations tailored to patient needs and beliefs.
• Assess needs of individuals seeking ACP assistance with a person-centered approach.
• Promote ACP as a standard component of healthcare through system-wide initiatives.
• Develop and deliver staff and community education on ACP.
