Job summary

Healing Partners is seeking a Wound Care Nurse Practitioner to lead clinical care across multiple Skilled Nursing Facilities in the region. This full‑time, day‑shift role involves travel to facilities, wound assessment, treatment planning, and collaboration with physicians and staff.

You will hold an active APRN license, complete a nurse practitioner program, and be committed to high‑quality patient care. Benefits include competitive salary and comprehensive coverage.

Qualifications

• Licensed APRN in the state with current registration.
• Master’s degree in Nursing and Nurse Practitioner program completion.
• Ability to obtain an APRN license if not currently active.

Responsibilities

• Plan, direct and evaluate resident and employee health care in long-term care.
• Daily travel to various facilities; regions vary by scheduling needs.
• Examine and treat wound-related illnesses and accidents.
• Interpret tests and recommend appropriate follow-up.
• Refer to physicians, write prescriptions, and perform bedside procedures.
• Educate patients and PCPs on wound management practices.
• Coordinate services to improve wound healing outcomes.
