Job summary

The Speech Pathologist employed by Waveny Care Network must travel throughout the Agency service area to provide skilled speech-language pathology services, perform comprehensive evaluations, and develop treatment plans in accordance with physician orders and agency policy.

Responsibilities include documenting in the electronic health record, collaborating with the interdisciplinary team, educating patients and families, and participating in care planning, hospice IDG meetings, and quality

Qualifications

• Master's degree in Speech-Language Pathology required.
• Connecticut licensure to practice Speech Therapy.
• Minimum three years of Speech Therapy experience; home care preferred.
• Demonstrates good verbal and written communication; works with an interdisciplinary team.

Responsibilities

• Accept referrals and make timely visits.
• Perform initial evaluations and ongoing reassessments; set measurable goals.
• Provide speech therapy per plan of care in consultation with physician.
• Document assessments, plans and visit data in a timely manner.
• Coordinate care with the patient health care team and discharge planning.
• Instruct patient/family and supervise home health aides when applicable.
