Job summary

Qualifications

• Current New York State License to practice Speech Pathology.
• Minimum of two years experience as a Speech Pathologist.
• Home Health experience is preferred.

Responsibilities

• Plan and provide speech/language and dysphasia rehabilitation in patients' homes.
• Work with patients and families to provide training and assistive devices.
• Document assessments, plans, goals, visit data, and billing information.
• Instruct patients, families and team members on programs to improve care.
• Travel to patients' homes and ensure regulatory compliance.
• Provide leadership and participate in continuing education.
