Job summary

Qualifications

• Master's Degree in Speech and Language Pathology.
• Current Connecticut licensure to practice Speech Therapy.
• Three years of Speech Therapy experience; home care preferred.
• Strong written and verbal communication skills.
• Ability to work with an interdisciplinary team.
• Knowledge of HIPAA and safety procedures.

Responsibilities

• Accept referrals and conduct timely initial evaluations.
• Perform comprehensive speech, language, and swallowing assessments.
• Develop and adjust treatment plans with physician orders.
• Coordinate equipment and discharge planning.
• Document assessments and visits within 72 hours.
• Communicate with patient, family, and healthcare team.
• Provide patient and family education.
