Qualifications

• Must hold a valid Speech Language Pathologist license in the state(s) of practice.
• Must possess or be eligible for ASHA Certificate of Clinical Competence (CCC) or be eligible to participate as an ASHA Clinical Fellow.
• Must have a valid driver's license or reliable transportation to patient residences.

Responsibilities

• Provide outpatient rehabilitation services in home/community settings for older adults.
• Evaluate and treat communication and swallowing disorders per facility and ethical guidelines.
• Coordinate care with interdisciplinary team and family to design and implement treatment plans.
• Document clinical treatments and progress timely and accurately.
• Supervise CFYs, students, and support personnel as required by state licensure.
• Promote safety and manage equipment and supplies for home-based therapy.
