Job summary

Healthforce is seeking a Speech Language Pathologist for an acute care travel assignment in Canaan, CT. The role runs 13 weeks with guaranteed 32 hours per week and a 5x8 day shift from 08:00 to 16:30, starting 11/30/2026.

This position focuses on assessments, treatment planning, and interdisciplinary collaboration in a nursing home setting. The ideal candidate will deliver direct SLP services, document progress, and support discharge planning while navigating travel requirements.

Qualifications

• Ability to work in a long-term care/nursing home setting.
• Day shift availability as specified (08:00–16:30).
• Strong documentation and communication skills.
• Ability to collaborate with an interdisciplinary care team.
• Willingness to participate in a 13-week travel assignment.

Responsibilities

• Conduct comprehensive speech-language assessments and screen for swallowing disorders in residents.
• Develop and implement individualized treatment plans addressing communication, cognitive-communication, and swallowing goals.
• Provide direct speech-language therapy and monitor progress; adjust plans as needed.
• Document assessments and treatment notes in the medical record; communicate findings to the care team.
• Collaborate with an interdisciplinary team to optimize patient outcomes and discharge planning.
• Educate residents and families on therapy goals, home practice programs, and safety strategies; participate in care conferences.
• Maintain equipment, follow infection control protocols, and ensure safety during therapy sessions.
