Job summary

Sweetwater Care in Montana seeks a licensed Speech-Language Pathologist to deliver comprehensive therapy for swallowing, speech and cognitive disorders in a SNF setting.

You will evaluate patients, develop and adjust treatment plans, document progress, and collaborate with a multidisciplinary team to optimize outcomes and ensure payer guidelines are met. This is a full-time, on-site role.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Provide professional speech-language therapy for swallowing, communication, cognitive and oral-motor disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Document evaluations, progress notes and discharge summaries for medical records.
• Collaborate with other therapists and physicians on patient treatment programs and progress.
• Ensure compliance with reimbursement systems and facility policies.
• Demonstrate courtesy, compassion and respect to patients and coworkers.
• Maintain compliance with the company’s Code of Conduct.
