Job summary

Sweetwater Care in Montana is seeking a Speech-Language Pathologist to assist patients with communication, swallowing and cognitive disorders in a SNF setting. Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to company guidelines.
• Provides professional treatment while using therapeutic techniques for rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Demonstrates knowledge to provide patient care appropriate in a SNF setting.
• Ddocument patient evaluations, treatment progress notes, and discharge summaries for medical records.
• Collaborate with other healthcare team members relative to patient treatment programs and progress.
