Job summary

Sweetwater Care in Montana is seeking a licensed speech-language pathologist to assess, diagnose and treat communication and swallowing disorders in a skilled nursing facility.

You will develop and implement individualized plans of care, document progress, and collaborate with physicians, therapists and staff to optimize patient outcomes while ensuring compliance with Medicare/Medicaid guidelines.

Qualifications

• License as a Speech-Language Pathologist and relevant credentials.
• Experience evaluating and treating communication and swallowing disorders.
• Ability to develop and document plans of care and progress notes.
• Collaboration with physicians, therapists and nursing staff.

Responsibilities

• Directly provide speech and language therapy to SNF patients.
• Administer swallowing assessments and modify treatment plans accordingly.
• Develop short- and long-term goals and monitor progress.
• Document evaluations, treatments and discharge summaries accurately.
• Coordinate care with the rehabilitation team and physicians.
