Amergis is seeking a School Occupational Therapist (OT) for a travel assignment in Tempe, AZ. The OT will evaluate students, develop and provide therapy services, and document care according to each student's plan of care. Requires active OT licensure in the state and 18+ years of age.

Travel position with weekly pay and comprehensive benefits. Minimum requirements include current ot licensure, CPR if applicable, and compliance with relevant professional standards.

Qualifications

• Current licensure as an Occupational Therapist in the State of practice.
• One (1) year of prior professional Occupational Therapy experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform student evaluations.
• Develop and provide therapy services.
• Document services in accordance with the plan of care developed for each student.
