Healthforce is seeking an experienced Travel Registered Nurse (RN) specializing in Long-Term Care for a contract in Dubuque, IA. You will deliver high-quality nursing care in a skilled nursing setting, managing chronic condition care plans, preventive health, and day-to-day wellness.

Qualifications

Experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF).
Proficiency with Electronic Health Record (EHR) systems.
Strong communication, time management, and documentation skills.

Responsibilities

Conduct comprehensive assessments and monitor residents for health changes.
Administer medications and manage IV therapy per physician orders.
Perform wound care and skin integrity assessments; implement pressure ulcer prevention.
Identify acute changes, update care plans, and escalate as needed.
Collaborate with physicians, therapists, and social workers for coordinated care.
Document all care and resident responses in the EHR accurately.
Educate residents and families about care plans, safety, and condition management.
