American Traveler is hiring an experienced Care Manager II for a hospital Case Management role requiring 2+ years of acute care case management experience and EPIC proficiency.

Requirements

• Active RN or LMSW license in the state of TX or compact required
• BLS certification preferred
• Case Management certification preferred
• Minimum of 2 years clinical experience required, with 1 year in acute care setting preferred
• Graduate of an accredited nursing program (BSN preferred) or MSW required, unless substituted by 5+ years demonstrated success in a Care Manager I role
• Required experience in Case Management, utilization review, MCG criteria, and InterQual criteria
• EPIC EMR experience required
• Acute hospital case management experience highly preferred
• Strong verbal and written communication skills with ability to interact with diverse populations
• Critical thinking, analytical skills, and ability to multitask in a fast-paced environment

Responsibilities

• Works within the hospital's Case Management department
• Manages a daily caseload of 20-25 patients in a high-pace environment
• Standard schedule of 5 days per week, 8 hours per day, generally 8:00 AM to 5:00 PM
• May be required to arrive at 7:30 AM and remain until all cases are completed for the day
• Collaborates with physicians, nursing staff, and multidisciplinary team members across the continuum of care
• Coordinate patient progression through the continuum of care and develop individualized care plans from admission through discharge
• Responsible for care coordination and discharge planning, including identifying post-acute care needs and resources
• Monitor length of stay and collaborate with interdisciplinary teams to remove barriers and expedite discharge
• Act as patient advocate, negotiating with payors, agencies, and vendors to coordinate resources
• Participate in multidisciplinary rounds and **escalate** cases to Physician Advisors or CM Director as needed
• Document in the medical record per regulatory and departmental guidelines
• May assist with special projects and serve as a preceptor to new associates
• Requires ability to move throughout the hospital for the majority of the workday, with some time spent in office setting
• Must maintain patient dignity and demonstrate cultural sensitivity in care delivery
