American Traveler is seeking an experienced Care Manager II for hospital case management in Texas. The role requires 2+ years in acute care case management and proficiency with EPIC EMR. Active RN or LMSW license in TX or compact is required, with BSN or MSW preferred.

The position involves coordinating patient progression, discharge planning, and collaboration with physicians and the care team in a high-paced hospital setting. BLS and CM certification are preferred.

Qualifications

• Active RN or LMSW license in TX or compact required.
• Strong verbal and written communication skills.
• Critical thinking and multitasking in fast-paced environment.
• EPIC EMR experience required.
• Graduate of accredited nursing program (BSN preferred) or MSW.

Responsibilities

• Manage daily caseload of 20–25 patients in a high-paced environment.
• Coordinate care progression from admission to discharge with the continuum of care.
• Collaborate with physicians, nursing staff, and multidisciplinary teams.
• Discharge planning and identifying post-acute care needs.
