American Traveler is seeking an experienced RN for a Home Health/Hospice Case Manager role in Maine. An active ME or compact RN license is required, with at least 2 years of home health/hospice experience. You will coordinate care for adult patients in home settings, managing 14–18 patients with a daily 5–6 visits schedule.

The position uses Epic EMR, includes an on-call rotation with overnights and some weekends, and requires a valid driver's license. A 13-week contract with benefits is offered.

Qualifications

• Active RN license (Maine or compact) required
• BLS certification required
• Minimum 2 years RN experience; at least 1 year in home health/hospice
• Epic EMR experience required
• Experience with PICC lines and related care preferred
• Experience with in-home infusion pumps
• Driver's License required for consideration
• Two professional references required

Responsabilités

• Provide case management in home health and hospice for adult patients
• Manage a caseload of 14-18 patients based on acuity with 5-6 visits per day
• Meet a minimum productivity of 25 units per week
• Day shift schedule with rotating weekends every third weekend
• On-call rotation including overnights and some weekends (~1x/week and 1 weekend/month)
• Utilize Epic EMR for documentation and care tracking
• Float policy of 25 miles between facilities as needed
• Carry agency laptop, iPhone, nursing supplies, and bags for assignments
• Case manage admissions, recertifications, and routine visits
• Independent self-scheduling; admissions experience preferred
• Dress code: business casual or scrubs; no jeans or open-toed shoes
