Responsibilities

• Deliver skilled nursing services to patients in their home setting under an approved plan of care.
• 13-week assignment with 8:00 AM to 5:00 PM day shifts.
• EPIC EMR used for charting.
• Float policy of up to 25 miles between facilities as needed.
• Case management responsibilities as assigned.
• Evaluate patients using subjective and objective findings per clinical standards and establish goal-oriented nursing plans of care.
• Administer nursing interventions and counsel, teach, and supervise patients and families in self-care techniques.
• Observe, evaluate, and report on patient status to the attending physician as needed.
• Coordinate care with other professional services involved in the patient's treatment.
• Participate in team communication, case conferences, and team meetings.
• Establish clear discharge plans and coordinate with the home care team.
• Apply and communicate skilled care and homebound requirements under Medicare Home Health Benefit or other payer guidelines.

• Local travelers are not eligible for renewals or extensions
