Job summary

American Traveler, LLC. is seeking an experienced RN Case Manager for a hospital Care Management department in Maine. The ideal candidate has 5+ years of hospital case management and acute adult nursing experience, with strong discharge planning and care coordination skills.

The role requires an active Maine or compact RN license, a driver’s license, and references. Schedule is 1st shift, weekdays with occasional weekends and holidays; local travelers may have reduced rates.

Qualifications

• Experience in discharge planning and care coordination required
• Driver's License required for consideration
• Two professional references required
• Flexible, easygoing problem solver with a calm demeanor and can-do attitude preferred

Responsibilities

• Work in the Care Management department of a 32-bed hospital unit
• Patient ratio of 1:20-24
• High acuity, complex patient population with a high percentage of behavioral health cases
• Uses EPIC, Wellsky, Careport/PASSR, Microsoft Teams, laptop, and Ascom phone
• Monday through Friday schedule on 1st shift, 8:00 AM to 4:30 PM
• Every third weekend required
• Holiday coverage required
• Floating required to cover Care Management department needs within a 25-mile radius between facilities
• No call requirements
• Responsibilities include discharge planning, care coordination, and case management for a high-acuity patient population
• Business casual dress code; any professional scrubs or casual professional attire is acceptable
• Candidates residing within 90 miles of any MaineHealth facility are considered local, with a reduced bill rate
• Local travelers are not eligible for renewals or extensions
• RTO should be kept to a minimum, with 3-4 days considered reasonable for a 13-week contract
• Candidates who have worked at any MaineHealth facility within the past 12 months should disclose this information
• AirPods, laptops, and cell phone usage are prohibited while on the unit
