Qualifications

Responsibilities

• Provide care coordination and discharge planning in an acute hospital setting.
• Work 5x8 day shift (8:00 AM–4:30 PM) with a 1:25 patient load.
• Occasional weekend work as needed; limited holidays.
• May float to sister facilities within the health system if required.
