Job summary

American Traveler Staffing Professionals is seeking an experienced RN Case Manager for an acute hospital setting in Oakland, CA. Requires 5+ years in RN case management and inpatient acute care experience with CA RN license and BLS.

The role involves care coordination, transitions planning, and collaboration with physicians, utilization managers, social workers, and bedside RNs. Travel assignment with a 13-week length and 40-hour weeks.

Qualifications

• Active CA RN license required.
• BLS certification required.
• 5+ years of RN Case Management experience required.
• Inpatient Acute Case Management (hospital) experience required.
• 2 supervisor references obtained within the past year required.

Responsibilities

• Care coordination and care transitions planning throughout the acute care patient experience.
• Collaborates with physicians, utilization managers, medical social workers, and bedside RNs to ensure timely progression and transition of patients to appropriate levels of care.
• Conducts patient assessments to determine readmission risk and screens for ancillary supportive services including Palliative Care.
• Reviews medical records to ensure patients meet level of care requirements and documentation supports LOC determination.
• Monitors length of stay and collaborates with financial counselors on inpatient stay denials.
• Formulates transition plans in collaboration with patients, families, physicians, and community-based support services.
• Screens 30-day readmissions and creates effective transition plans with the interdisciplinary team.
• Identifies ED high utilizers and makes appropriate care plans and community resource referrals.
