Job summary

American Traveler in Eureka, CA seeks an experienced RN Case Manager to coordinate inpatient care, oversee discharge planning, and ensure compliance with CMS, DRG, HIPAA, and Joint Commission guidelines. The ideal candidate has a CA RN license, at least 2 years of acute hospital CM experience, and prior travel nursing exposure.

Epic EMR is required; duties include care coordination and prior authorizations. Local candidates within 50 miles preferred.

Qualifications

• Active CA RN license required.
• Minimum 2 years acute hospital Case Management experience.
• Travel nursing experience required; first-time travelers may be considered with approval.
• Epic EMR experience required.
• Experience with discharge planning, care coordination, authorizations.
• Knowledge of CMS, DRG, HIPAA guidelines and Joint Commission standards.
• Two supervisory references in the last 3 years.
• Full SSN and DOB required for submission.

Responsibilities

• Coordinate care in Acute hospital Case Management/Utilization Review.
• Day shift schedule 8:00 AM to 4:30 PM; every other weekend.
• Must float between units and sister facilities as needed.
• Epic charting system used for documentation.
• Responsibilities include care coordination, discharge planning, DME ordering, and prior authorizations.
• Local candidates within a 50-mile radius.

Requirements

• Active CA RN license required
• Minimum 2 years of acute hospital Case Management experience performing care coordination and discharge planning
• Prior travel nursing experience required, though first-time travelers may be considered with clinical manager approval
• Epic EMR experience required
• Experience with admission criteria, appeals and denials, benefits eligibility, care coordination, discharge planning, needs assessment/DME ordering, and prior authorizations
• Knowledge of CMS, DRG, HIPAA guidelines, and The Joint Commission/Core Measures/National Safety Goals
• Two supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting
• Full SSN and DOB required for submission

Responsibilities

• Acute hospital Case Management/Utilization Review department
• Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
• Required to work every other weekend
• No on-call responsibilities
• Floating between like units required based on facility need
• Epic charting system used
• Responsibilities include care coordination, discharge planning, needs assessment, DME ordering, and prior authorizations
• May float between sister facilities near the contracted location as needed
• Local candidates accepted within a 50-mile radius
• Former permanent staff may return after a 3-month waiting period, provided they reside outside a 50-mile radius of the facility
• Candidates must disclose any prior work history with the health system, including permanent, travel, or per-diem roles, with location and dates
• All requested time off must be submitted at the time of application for consideration
• Located in Eureka, California, offering easy access to coastal areas and several state and national parks
