AMN Healthcare Revenue Cycle is seeking a Travel Nurse RN Case Manager in Chico, California for a 13-week assignment. This role focuses on coordinating patient care across the episode of illness, collaborating with families, physicians and payors. 40 hours/week, 8-hour shifts, on-site positioning.

Requirements include CA RN license, CPR/BLS, and 1+ year of acute care case management experience. Epic is used for clinical documentation and workflow management during the assignment.

Qualifications

• CA RN license is required.
• CPR / BLS certification is required.
• 1+ year of recent Case Management experience in an acute care setting.

Responsibilities

• Coordinate care for an episode of illness or treatment with patient, family, doctors and payors.
• Lead care planning using nursing outcomes and evidence-based pathways.
• Assess, plan, intervene and evaluate using the nursing process.
