Responsibilities

• Deliver coordinated nursing care for patients in a home hospice setting according to established standards and the nursing process.
• Assess patient data, identify care needs, and develop and implement care plans; adjust plans as needed.
• Collaborate with patient, family, and multidisciplinary team to ensure safe, compassionate, goal-directed care.
• Educate patients and families, provide health information, and participate in discharge planning to support continuity of care.
• Direct and coordinate duties of the nursing team, delegating tasks while maintaining safety and quality standards.
• Document care delivery and outcomes, and contribute to ongoing evaluation of patient progress.
