The ideal candidate holds a Georgia RN license, has at least 1 year of clinical nursing experience, and demonstrates strong communication skills to counsel patients and families.

Qualifications

• Graduate of an accredited nursing program.
• Georgia RN licensure required.
• Minimum of 1 year clinical nursing experience.
• Ability to work independently and coordinate with care teams.
• Excellent interpersonal, written, and verbal communication skills.

Responsibilities

• Provides services according to the plan of care.
• Conducts initial evaluation visit and ongoing reevaluations.
• Initiates and revises the plan of care as needed.
• Performs preventive and rehabilitative nursing procedures.
• Prepares and documents patient visits and care conferences.
