Job summary

Qualifications

• 2+ years of Physical Therapist experience
• LTC experience preferred
• Prior travel experience preferred

Responsibilities

• Assess, differential diagnose, and identify deficits in movement and daily activities.
• Develop an individualized treatment plan with patient-centered goals to restore function and reduce pain.
• Perform patient evaluations and select tests and measures to obtain data.
• Implement treatment techniques addressing impairments identified in evaluation.
• Reassess patient progress and adjust plan as needed.
• Initiate and manage discharge planning from first visit through treatment.
• Supervise and delegate to ancillary staff and students per state laws.
• Complete clear, accurate documentation within policy timelines.
• Educate patient and family to meet needs and ensure safety.
