Job summary

Healthforce is seeking a licensed Physical Therapy Assistant for a LTAC travel assignment in Winchester, New Hampshire. Start date 11/02/2026 for a 13‑week engagement with guaranteed 35 hours per week and a 5x8 shift from 08:00 to 16:30.

You will implement and monitor plans of care under a PT, lead therapeutic exercises, assist with daily living activities, and ensure accurate documentation while maintaining patient safety in a long‑term care setting.

Qualifications

• Active Physical Therapy Assistant license in New Hampshire or eligible.
• Experience in long-term care / LTAC settings is preferred.
• Proficient in therapeutic exercise, gait training, ROM, and functional mobility.
• Strong communication and teamwork; accurate documentation.
• Available for a 13-week assignment with guaranteed hours.
• Travel-capable and able to work a 5x8 day shift.

Responsibilities

• Implement and monitor PT plans of care under supervision of a physical therapist for patients in a long-term acute care/long-term care setting.
• Lead and progress therapeutic exercises, ROM, strengthening, gait training, balance, and functional mobility activities.
• Assist with activities of daily living and functional training to support patient independence.
• Monitor patient responses to treatment, document progress, and communicate effectively with the PT, nursing staff, and other team members.
• Maintain patient safety, proper positioning, wound care as directed, modality applications per orders, and equipment safety.
• Ensure accurate, timely charting and adherence to infection control and safety protocols.
