Healthforce is seeking a Travel Physical Therapist specialized in geriatric rehabilitation for a skilled nursing facility in Rochester, NH. The assignment runs 13 weeks starting 11/23/2026 with a weekly pay range of $2,343–$2,409 for 35 hours per week, scheduling 08:00–16:30, five days weekly.

The role involves developing resident-specific plans, delivering therapy, and coordinating with the care team. Credentialing and onboarding support are provided by Healthforce during the assignment.

Qualifications

• NH license or eligibility to practice in NH.
• Willing to commit to a 13-week travel assignment in Rochester, NH.
• Experience in skilled nursing or geriatric rehab preferred.
• Strong communication and teamwork skills.

Responsibilities

• Assess residents' PT needs and develop care plans with the care team.
• Provide direct therapy: exercise, gait and balance work, mobility, strength, and functional activities.
• Apply evidence-based techniques suitable for older adults in SNF.
• Reassess progress, document notes, and adjust plans to meet goals.
• Collaborate with physicians, nursing staff, and therapy assistants; communicate changes.
