Qualifications

Current PTA license in New Hampshire or eligibility to obtain licensure in NH.
Ability to work effectively in LTAC/long-term care rehab setting under supervision of a physical therapist.
Solid communication skills and ability to document clearly and concisely.

Responsibilities

Provide physical therapy services to patients under supervision of a licensed PT in a LTAC/long-term care rehabilitation setting.
Assess patient status and implement, monitor, and adjust individualized treatment plans including therapeutic exercise, gait training, balance, and ROM.
Document patient progress, outcomes, and any changes in plan of care; communicate findings to the interdisciplinary team.
Remain compliant with safety protocols, infection control, and device/orthotic use as indicated; supervise and assist with modalities as permitted.
Support discharge planning and provide patient and family education to promote continuity of care and home program adherence.
Maintain appropriate equipment, organize therapy space, and contribute to a positive, collaborative rehab environment.
