Job summary

Preferred Home Health Care & Nursing Services in Fairfield, CT seeks a Physical Therapist to assess patients and deliver rehabilitation services in a home health setting. You will develop care plans, deliver interventions, monitor progress, and document conditions.

Collaborate with a multidisciplinary team, adjust treatments as needed, and educate patients and families on therapy methods. CPR and licensure are required, with NPTE eligibility or completion.

Qualifications

• Must have DPT or BPT with NPTE and current state license.
• Minimum of 1 year of experience.
• CPR certification required.

Role:
