Role: The Physical Therapist (PT) is responsible for evaluating referred patients requiring rehabilitation services, developing individualized treatment plans, and providing skilled therapy interventions in accordance with physician orders.
Responsibilities: Assess patient needs, set realistic goals, document progress, and educate patients and families on therapy methods. Collaborate with multidisciplinary teams to plan and evaluate care, ensuring goals are met and care is modified as needed.
Qualifications: Must hold a Doctorate or Bachelor's degree in Physical Therapy from an accredited institution, pass the NPTE, and have at least one year of experience. Current state licensure or active application process, along with CPR certification, are required.
Skills & Key Duties: Provide direct patient care, develop and modify care plans, assess patient progress, and document medical conditions. Must work under physician supervision, establish achievable goals, and educate patients and families effectively.
