Job summary

Northshore Home Health Care is seeking a dedicated Physical Therapist to assess, plan, and deliver rehabilitation services in a home health setting, developing individualized care plans and educating patients and families.

The role requires a Doctorate or Bachelor's in Physical Therapy, NPTE, current state licensure or active application, CPR certification, and at least one year of experience. Day shift, full-time with comprehensive benefits.
