Proforma Health Inc. is seeking an outpatient travel Physical Therapist to provide therapy across clinics, including ortho, dry needling, vestibular, laser, and shock wave modalities. You will manage a patient load of 12–14 daily and maintain high productivity.

• Active State License required, BLS preferred.

Float to different clinics as needed, manage a patient load of 12-14 daily, and maintain productivity of 60 visits per week. Collaborate with team members, ensure quality patient care, and follow policies on follow-up visits and overtime approvals.

Qualifications & Skills:

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution. Active State License required, with BLS certification preferred. No PTA accepted. Strong communication skills, adaptability, and ability to work independently are essential.
